Privacy Policy

This policy explains what information Slim Wellness Center collects through our website and services, how we use it, and the choices you have.

Last updated: July 28, 2026

Two separate privacy documents

This Privacy Policy covers information we collect through our website and general business operations — things like contact forms, quiz responses, appointment requests, and site analytics.

If you become a patient, your medical records and treatment information are protected health information (PHI) governed by HIPAA and California law. Those are covered by our separate Notice of Privacy Practices, which you will receive when you become a patient and which controls in the event of any conflict with this policy.

1. Who we are

Slim Wellness Center is a medical wellness clinic located at 655 Euclid Ave #210, National City, CA 91950. In this policy, "we," "us," and "our" refer to Slim Wellness Center. "You" refers to any visitor to our website or person who contacts us.

2. Information we collect

Information you give us

  • Name, email address, phone number, and mailing address
  • Responses you provide in our online quiz or intake forms
  • Appointment requests and scheduling information
  • Messages you send us by form, email, or text
  • Health-related information you choose to share with us before becoming a patient

Information collected automatically

  • IP address and approximate location
  • Browser type, device type, and operating system
  • Pages you visit, links you click, and time spent on the site
  • The website or ad that referred you to us
  • Information collected through cookies and similar technologies

Information from third parties

We may receive information from advertising platforms, analytics providers, and scheduling or payment partners that work on our behalf.

3. How we use your information

  • To respond to your questions and requests
  • To schedule, confirm, and follow up on appointments
  • To determine whether our services may be appropriate for you
  • To send appointment reminders and service-related messages
  • To send marketing communications, where you have consented
  • To operate, secure, and improve our website
  • To measure the performance of our advertising
  • To comply with legal and regulatory obligations

4. Text messages and calls

If you provide your phone number and consent, we may contact you by phone call or SMS text message about appointments, treatment, and — where separately authorized — promotions. Message and data rates may apply, and message frequency varies.

You can opt out of text messages at any time by replying STOP. Reply HELP for assistance. Opting out of marketing messages does not stop transactional messages related to care you are actively receiving. Consent to receive marketing texts is not a condition of receiving treatment.

5. How we share information

We do not sell your personal information. We share it only as described here:

  • Service providers. Companies that operate our website, CRM, scheduling, email, text messaging, analytics, and payment processing, under contracts limiting their use of your information.
  • Pharmacies and laboratories. Where necessary to fill a prescription or process lab work as part of your care.
  • Financing partners. If you choose to apply for financing, you interact directly with Cherry or CareCredit under their own privacy policies.
  • Legal requirements. Where required by law, subpoena, or to protect the rights, safety, or property of any person.
  • Business transfers. In connection with a merger, acquisition, or sale of assets, subject to applicable law.

6. Cookies and tracking

We use cookies and similar technologies to operate the site, understand how it is used, and measure advertising. For details and your choices, see our Cookie Policy.

7. Your California privacy rights

If you are a California resident, the California Consumer Privacy Act, as amended by the California Privacy Rights Act, gives you the following rights regarding personal information that is not otherwise governed by HIPAA or the California Confidentiality of Medical Information Act:

  • Right to know what personal information we collect, use, and disclose
  • Right to delete personal information we hold about you, subject to legal exceptions
  • Right to correct inaccurate personal information
  • Right to opt out of the sale or sharing of personal information for cross-context behavioral advertising
  • Right to limit the use and disclosure of sensitive personal information
  • Right to non-discrimination for exercising any of these rights

To exercise a right, contact us using the details below. We will verify your identity before responding. You may designate an authorized agent to act for you.

Note that medical information governed by HIPAA and the California Confidentiality of Medical Information Act is subject to those laws rather than the CCPA, and requests about your medical records should be directed to us under our Notice of Privacy Practices.

8. Data security

We maintain administrative, technical, and physical safeguards designed to protect your information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. Please do not send sensitive medical details through unsecured email or website forms.

9. How long we keep information

We retain personal information for as long as needed for the purposes described in this policy, and as required by law. Medical records are retained according to applicable California and federal record retention requirements.

10. Children

Our website and services are intended for adults 18 and older. We do not knowingly collect personal information from children under 13. If you believe a child has provided us information, contact us and we will delete it.

11. Third-party websites

Our site links to third-party sites, including our financing partners and social media profiles. We are not responsible for their privacy practices. Review their policies before providing information.

12. Changes to this policy

We may update this policy from time to time. When we do, we will revise the "Last updated" date above. Material changes will be communicated as required by law.
